Polymeric nanotherapeutics: clinical development and advances in stealth functionalization strategies

Summary

This review surveys the clinical development of long-circulating polymeric nanotherapeutics and the surface strategies used to make synthetic carriers less visible to biological clearance systems. Such carriers can improve cargo solubility and pharmacokinetics, while tuning their properties may increase localization at disease sites and reduce unwanted exposure. The authors compare advances in “stealth” functionalization, including different stealth polymers described in the literature, and place these approaches alongside newer biologically inspired particles that present markers of “self.” The principal contribution is to connect clinical progress with a key design problem: maintaining circulation without compromising cargo delivery, safety, or manufacturability. The review also distinguishes conventional polymer-based camouflage from biomimetic self-recognition as conceptually different routes to the same objective. However, the abstract does not list products, trial phases, search criteria, comparative pharmacokinetic values, clinical outcomes, or adverse events, and it does not identify a universally superior stealth material. Polymer immunogenicity, accelerated clearance after repeat dosing, heterogeneity of biological coatings, scale-up, quality control, and regulation remain important uncertainties. The article therefore maps development strategies but does not establish that any one functionalization reliably improves patient outcomes.
